The purpose of the Risk Management Division is to foresee and control, to the extent possible, any liability to the City. In the event a liability occurs, the aggrieved individual may file a claim by submitting a claim form
to the City Clerk’s Department
A claim must be filed with the City Clerk of the City of Fontana within six months after occurrence of the incident or event on which the claim is based. Be sure your claim is against the City of Fontana, not another public entity.
Where space is insufficient, please use additional paper and identify information by paragraph number.
