Registration Requirements
All City of Fontana Pee Wee Sports Program participants are required to submit the following information at the time of registration:

• Birth certificate
• Program payment
• Signed parent's oath
• Waiver

Program registration will not be accepted without all required information. Registration is taken on a first-come, first-served basis at all City of Fontana Community Centers.
