Evidence of Child's U.S. Citizenship
Provide one of the following types of documentation:
Born in the United States
- Certified U.S. birth certificate
- Previous fully valid U.S. passport
Born outside the United States
- Report of Birth Abroad (Form FS-240)
- Previous full valid U.S. passport
- Certificate of Citizenship for Naturalization from INS
- Certificate of Birth Abroad (Form DS-1350)
- Other evidence for first-time documentation
Parental Identification
Provide one of the following types of documentation:
- Valid drivers license
- Valid official U.S. Military ID
- Valid U.S. Government ID
- Valid U.S. or foreign passport with recognizable photo
- Naturalization / Citizenship Certificate from INS with recognizable photo
- Alien Resident Card from INS

Evidence of Child's Relationship to Parents or Guardian
Provide one of the following types of documentation:
- Adoption decree including adopting parent(s)' names
- Court order establishing custody
- Court order establishing guardianship and authority to apply for passport
Born in the United States
- Certified foreign birth certificate including parent(s)' names
- Certified U.S. birth certificate including parent(s)' names
Born outside the United States
- Certified foreign birth certificate including parent(s)' names
- Report of Birth Abroad (Form FS-240)
- Certificate of Birth Abroad (Form DS-1350) with parent(s)' names
