Citizenship, Relationship & Identification Documentation

Evidence of Child's U.S. Citizenship


Provide one of the following types of documentation:

Born in the United States
  • Certified U.S. birth certificate
  • Previous fully valid U.S. passport

Born outside the United States
  • Report of Birth Abroad (Form FS-240)
  • Previous full valid U.S. passport
  • Certificate of Citizenship for Naturalization from INS
  • Certificate of Birth Abroad (Form DS-1350)
  • Other evidence for first-time documentation

Parental Identification


Provide one of the following types of documentation:
  • Valid drivers license
  • Valid official U.S. Military ID
  • Valid U.S. Government ID
  • Valid U.S. or foreign passport with recognizable photo
  • Naturalization / Citizenship Certificate from INS with recognizable photo
  • Alien Resident Card from INS

Evidence of Child's Relationship to Parents or Guardian


Provide one of the following types of documentation:
  • Adoption decree including adopting parent(s)' names
  • Court order establishing custody
  • Court order establishing guardianship and authority to apply for passport

Born in the United States
  • Certified foreign birth certificate including parent(s)' names
  • Certified U.S. birth certificate including parent(s)' names

Born outside the United States
  • Certified foreign birth certificate including parent(s)' names
  • Report of Birth Abroad (Form FS-240)
  • Certificate of Birth Abroad (Form DS-1350) with parent(s)' names
